To obtain an electronic, printable Certificate for Cardinal Health medical products, enter the Cardinal Health product number (as ordered) and the product lot number (as it appears on the product labeling) into the fields below. Select the type of certificate desired in the drop down and click the 'print and preview' button to generate the certificate.

NOTE: All fields are required to generate the certificate, and only one product and lot can be generated at a time.
